Finally got this fixed.
It was, as I expected, a problem at our ISP (BT). To be specific it was caused by faulty hardware at the exchange.
Since they repaired the fault we've not had any problems.
By far the toughest part of this problem was trying to get BT to acknowledge that the problem was at their end. Slugged it out though and got there in the end!
