There are back doors for Vista and 7 that are easier and quicker to hack than ophcrack.

or just try http://pogostick.net/~pnh/ntpasswd/ that one works well as long as it isn't a domain password. Only Offline Passwords.

+1 for ntoffline. Just cleared a password yesterday using it when neither kon-boot or ophcrack would work.
